Annual Education Update

PLEASE READ ALL INSTRUCTIONS ON THIS PAGE!

In order to comply with regulatory requirements and to ensure that we offer our patients the most up-to-date care, all employees are required to complete the Annual Educational Update by May 31, 2026.

Once you have reviewed the slides, you may begin your test below. Type your first name and last name as it appears on your paycheck. The password is your personal employee number assigned through payroll. You can also add your email address if you would like to receive a copy of the test results. The results will be sent to Lynn Whittle.

Once you have completed the test, you must electronically sign the Annual Education Update certification.

You must also electronically initial and sign the Confidentiality & Non-Disclosure Form.
